1. I/We acknowledge that attendance at courses may involve activities that are very dangerous and involve the risk of serious bodily injury and/or death and/or property damage.
2. I/We further acknowledge that being present on the premises of SLEO before, during, and after the school’s standard hours of operation, as well as special events, field trips and off-campus activities, and using the equipment and facilities offered by SLEO may involve the risk of serious bodily injury and/or death and/or property damage caused the acts of third-persons known and unknown to the teachers, agents, officers or directors of SLEO.
3. I/We, our heirs, personal representatives and assignees do hereby release, waive, discharge, and covenant not- to-sue SLEO, its teachers, agents, officers or directors, whether current, past or future, from any and all liabilities or claims for any and all injuries, including but not limited to death, paralysis, or damage to property, arising out of or allegedly caused by the minor participant’s participation in courses offered at SLEO, the minor participant’s presence on premises offered by SLEO for such courses, including but not limited to special events, field trips and/or off-campus activities, or the minor participant’s use of equipment or facilities offered by SLEO. I/we, our heirs, personal representatives and assignees further acknowledge and agree that this release, waiver, discharge and covenant not-to-sue expressly releases SLEO from any liability or claim that arises from or is allegedly caused in whole, or in part, by the negligence of any teacher, agent, officer or director of SLEO, or any third-persons, known or previously unknown to SLEO, its teachers, agents, officers or directors.
4. I/We hereby acknowledge and agree that the minor participant’s attendance at SLEO courses and electives are voluntary and that I/we knowingly assume all such risks.
5. I/We have read this waiver of liability, fully understand its terms, and understand that I am giving up substantial rights, including my/our right to sue. I/We acknowledge that I/we am/are signing freely and voluntarily, have had an opportunity to consult with an attorney prior to signing this agreement, and intend by my/our signature to bring about a complete and unconditional release of all liability, including the negligence of SLEO, its teachers, agents, officers and directors to the greatest extent allowed by law.
6. I/We hereby consent to SLEO’s collection, use, and disclosure of information about my children through the course registration process and other manual or electronic procedures of the SLEO and course/application providers. I understand that my child (ren) is (are) attending online/onsite courses and/or using online applications, and that the information collected is needed for both course registration and progress reporting. This consent form covers all types of online/onsite courses and/or applications, included but not limited to video courses and web- based courses. Your child's image may be transmitted during video portions of the online/onsite courses. The information supplied to the course provider and SLEO may include, but not be limited to: student's name, address, phone number, age, gender, grade, email address, and learning progress. Since the online courses applications are web-based, content accessed from home or elsewhere outside the school could put the student in contact with objectionable materials via the internet. It is the responsibility of the parent/guardian of the student to restrict any access to materials deemed inappropriate by the parent/guardian.
7. I/we hereby acknowledge and agree that the minor participant’s attendance of the online/onsite course offered by SLEO. The participant and the guardians are agreed to follow all rules and procedures set forth by SLEO regarding online/onsite classes. I further agree that attending offsite courses are voluntary and that I/we knowingly assume all such risks.
